Person-Centered Planning: Beyond the Template

Person-Centered Planning: Beyond the Template is for service providers and Service and Support Administrators, also called SSAs. This training will be facilitated by DODD’s Office of Policy and Strategic Direction and Office of Provider Standards and Review.

Why a new training on person-centered planning?

Going “beyond the template” means SSAs and providers are active participants, working together to create effective person-centered plans. This training introduces a consistent way to look at person-centered planning through real-life examples of individual service plans developed by teams in 2017 and other resources.

You will leave with tools to evaluate the work you’ve done and how your agency is doing when it comes to person-centered planning, along with a greater understanding of working together as a team.

Time will be set aside to develop clear action plans on how you can get to the next level.

Going forward, you will have access to a website of tools and resources to improve your practice in the seven areas, in addition to ongoing webinars, resources, and new contacts at other agencies who are doing great work.

 

We suggest packing your lunch as there will only be a 45-minute break. DODD CPDs will be available.
